Am using the latest official extension but on check out page, No fields displayed for user to input their payment details.

On placing the order, it redirects to the following blank url: pesapalexpress/payment/redirect/

I have followed the procedure correctly, uploaded the correct code to the correct folders, set up my consumer key & secrets, set up my ipn.

Am running Magento 1.9.1.0

What is the issue?

The most likely issue is that the extension is not compatible with the current Magento which explains the blank pages because of fatal errors.
